Yearly Return Visit (IN-PERSON)
This appointment is for established patients booking their yearly follow up appointment.
Service Description
Please let me know prior to your appointment if 1. Insurance has changed and if there are any labs that you do not want drawn or if there are any labs that you do want drawn that were not drawn last year. 2. If you are on Testosterone and or Thyroid medications please be sure to take your medications and wait at least 4 hours to have labs drawn 3. All labs are fasting 4. If you are still having cycles be sure to do lab work AFTER ovlation
Cancellation Policy
To cancel or reschedule, please contact us 24 hours prior to your scheduled appointment. Missed appointments, without communication are subject to a $50 fee.
