The concept that “you don’t need to be sick to get better” is at

the forefront of my health and wellness membership.

A health and wellness membership program provides efficient and connected access to your health care provider. This ensures not only I know you and your goals, but you feel comfortable with me. Being open and honest with each other creates an environment that will empower you to achieve the best version of yourself.

Frequent and small changes every day towards positive results will make you healthier.

My health and wellness membership sets you up for success by properly equipping you with tools intended to help you build your self-efficacy, as opposed to merely throwing health and wellness education at you and leaving you to your own devices. 

 

Whether you are looking for preventive health care or need to know you have access when you need access, my Health and Wellness Membership will help you clear the barriers that make it so difficult to access proper health care access and information.  

Pricing

Monthly membership cost: $150

Annual membership cost:  $1620

 

If membership is canceled prior to 12 months term, you are responsible for paying the remainder of the annual contract or the difference between services already rendered and payments already made. Clients who cancel membership will not be able to re-join the clinic for one year.

What is included in your membership:

Preventative Care:

​

  • Preventative care: Annual Well women exam

  • Contraceptive Counseling and management

  • Yearly physical exams

  • Weight loss management (cost for medication not included)

  • Health and Wellness Support and Direction

  • Lab work interpretation

 

 

Problem visits:

​

  • Urinary Tract Infection diagnosing and treatment

  • Vaginal infections diagnosing and treatment

  • STI diagnosis and treatment

  • Pelvic Pain diagnosing and treatment (referral for ultrasound cost not included)

  • Abnormal pap (referral may be indicated with abnormal pap’s)

  • Abnormal uterine bleeding evaluation

  • Uterine Biopsies

  • Irregular cycles

  • Hormone consultations, PMS

  • IUD Placement and removal (fee for IUD not included)

  • Nexplanon removal and replacement (fee for Nexplanon not included)

 

 

Yearly lab work with comprehensive review includes the following labs:

​

  • Follicle Stimulating Hormone (FSH)

  • Estradiol

  • Testosterone

  • Complete Metabolic Panel (CMP)

  • Complete Blood Count (CBC)

  • Vitamin D

  • Vitamin B12

  • Full Thyroid Panel (TSH, T3, TPO, T4)

  • Lipid Cascade Panel with Cholesterol, LDL, HDL, Triglycerides

  • Fasting Insulin level

  • AM Cortisol Level

  • C- Reactive Protein (CRP)

  • Total Iron Binding Capacity/Iron/Ferritin

  • Hbg A1C

  • Pap Smear (one yearly)

    • Additional blood work at reduced rate
