Cancellation Policy

Prior to your appointment I will be sending you the Health Insurance Portability and Accountability Act (HIPPA) forms for you to fill out as well as a brief health history form. I am available to answer any questions you may have to help make the most of our time together. If you are unable to attend an appointment, please try to cancel at least 24 hours beforehand or contact me and let me know you need to cancel, so we can reschedule. All no-show appointments will be charged the full rate of the session.
